BACKGROUND CHECKS

Most important are the CORI and fingerprinting forms. It is a condition of employment that all newly hired employees must be fingerprinted prior to the start of their assignment.

Employment & Payroll Forms

As a new employee, there are several forms that are required before the hiring process can be completed. The files below include all the up-to-date forms that are required for employment and payroll purposes for specific roles. You may print and complete the full packet at home or come into the office. All forms must be submitted in person to the HR department in central office at 566 Main Street. Be sure to bring two (2) or more forms of ID to complete the CORI and I-9 Verification. Acceptable forms of ID are: Driver's License, Social Security Card, Birth Certificate or US Passport.
