ATTENTION 6TH GRADE PARENTS
Students entering 7th grade are required to have a booster dose of Tetanus Vaccine or Tdap Vaccine.
Also, be advised that students are required to submit one physical exam while attending the middle school.
Please contact your child's physician and make arrangements for him/her to receive their immunization and or physical exam before the start of the '08-'09 school year.
Send physician documented dates to the Health Office before the start of the school year.

Our goal in the health office is to see that students at NAMS are enjoying optimum health.
Reasons for Dismissal / Exclusion
1. Temperature over 100 degrees (oral temperature)
2. Cough with fever
3. Suspected infection of eye, ears, nose, throat, skin scalp (unless under physician’s care for 24 hours)
4. Severe headache or abdominal pain
5. Suspected communicable disease and/or rash of unknown origin
6. Suspected fracture or any severe or disabling injury
7. Questionable need for sutures
8. Vomiting or diarrhea
9. Severe head injury
10. Suspected pediculosis (head lice)
Please keep us apprised of any serious injuries or illnesses along with any contagious illnesses such as Strep Throat.
One physical exam is required while your child attends middle school. Also, prior to entering 7th grade, students are required to show documentation that they have received a tetanus booster if the last one is more than 5 years ago.
Students are not permitted to carry their own medications except for the following:
1. Students with diabetes are permitted to keep all their diabetic supplies with them.
2. Students with asthma are permitted to carry their inhalers with them.
3. Students with severe allergies are permitted to carry their epi pens with them.
4. Students with Cystic Fibrosis are permitted to carry their enzymes with them.
Please send to the health office the necessary medical orders from your child’s pediatrician when they are prescribed medication for any of the above mentioned reasons or if they will be needing to take medication in the health office during the school day.
